The call you make when the usual answer doesn't work.

We know you already have agencies you trust. We’re asking to be the one you call when a patient could go home today if someone were there, when a family is overwhelmed by a new diagnosis, or when an adult child three states away is calling your office every week.

What you can count on

A callback within two hours of your referral during business hours. After hours, the on-call phone is answered by a member of our office team, not an answering service.

Care starts within 48 hours of the in-home visit, often sooner for hospital discharges.

A written update after the first week, with the family's consent, and a monthly report for clients in our Care Oversight program.

The same caregiver, as much as humanly possible. Our caregivers are employees with guaranteed hours, which is how we keep the same face at the door.

Honest answers. If we can't staff it, we'll say so on the first call and help you find someone who can.

Who we serve well

Hospital and skilled nursing discharge planners. Our Hospital-to-Home transition covers the 30 days after discharge, starting with a caregiver at the house when the patient arrives: a home safety walk-through, medication reminders from the discharge paperwork, meals, bathing, transfers, follow-up appointments, and an observation log that comes back to you.

Physicians and practice staff. Families ask you where to turn. Send them to us and we'll handle the intake, the in-home assessment and the care plan, and keep you informed of the changes you'd want to know about.

Hospice and palliative teams. We work alongside your nurses and aides, covering the hours between visits so the spouse can sleep. We coordinate with your team and never duplicate it.

Elder law attorneys, financial planners and trust officers. Care Oversight is built for the client whose family isn't nearby: a monthly in-home visit by a care coordinator and a plain-English written report to the family and, with consent, to you. Many long-term care insurance policies cover our hourly care; we'll help the family with the claim.

Senior living and memory care communities. For residents who need more hands than your staffing allows, or families who want to try home first, our 90-Day Dementia Starter gives them a plan and a trained caregiver.

How to refer

Call us, email us, or use the form below. Tell us the patient’s name, where they are now, the expected discharge or start date, the county, who the family contact is, and anything you’d want us to know before we walk in. We’ll take it from there and call you back within two hours.

What we don't do

We are a non-medical home care agency. We don’t provide skilled nursing, therapy or medication administration, and we’ll never pretend to. If a patient needs home health, we’ll say so and coordinate with the agency you choose.

Who you'll work with

UniCura was started by a Morgantown healthcare executive who spent fifteen years building and running care businesses, and who is also the son coordinating his own mother’s care. Our Founder reviews every referral personally and meets every family before care begins. Our Care Transitions lead will be in your building regularly; until then, every referral comes to the Founder directly.
