The monthly report: a short reassurance summary at the top ("here is what you need to know this month"), with visit-level detail behind it. Shared with the family and, if the family chooses, with their advisor.
Early flags: a call the same day if something changes, so the family decides before it becomes an emergency.
A smooth path into hourly care or the Hospital-to-Home program if and when it is needed.
