UniCura Health’s Hospital-to-Home program provides fast-start, non-medical in-home support for seniors in Morgantown and North Central West Virginia coming home from a hospital, rehab facility, or nursing home: a two-hour callback, care in the home within 48 hours, and 30 days of structured support through the period when readmissions and falls are most likely.
we take the call, confirm the start time, and can meet the family at the hospital or at home on day one.
Extra hours to get the person settled, the house set up, prescriptions filled, and the discharge instructions turned into a daily routine.
scheduled visits at the tier level that fits (Personal Care is most common) covering medication reminders, meals and grocery restock, rides to follow-up appointments, a fall-risk walkthrough of the home, and help with exercises prescribed by physical therapy.
coordination with the home health agency, PT, or hospice if one is involved, and a short update to the family after every visit.
a review call. Many clients step down to a lighter schedule; some continue; some no longer need us.
